I always like to go out of a textbook. I start with theory, and then accustom students to different difficulties of applying that theory to make sure solving questions becomes second nature to them. Going through past exam papers is also really important to make sure kids can apply knowledge in an exam setting.
I've been giving lessons to primary school and junior high school children of different levels of ability. I know how to cater lessons to the needs of individual students, and in high school, I did a lot of work tutoring my peers too :)
I'm always willing to personally negotiate fees, times, or any other details with each student and family. Everyone is different, so my approach towards everyone is catered to them :)
