I try to make the concepts clearer and understand what the students think to focus lessons accordingly. The lesson structure varies depending on the purpose of the lesson (revision, start a topic from beginning, practising harder problems, clearing concepts, etc). Generally, basic definitions and easy problems are covered followed by harder problems if the concepts are new.
